Azria Health Prairie Ridge ranked as the second largest nursing home in Des Moines County for the first quarter of 2026, as reported by the Centers for Medicare and Medicaid Services (CMS).
According to available data, the facility provided 62 beds, which is 32 fewer than the Des Moines County average for the period. The center cared for an average of 59.5 residents during the quarter.
The CMS gave Azria Health Prairie Ridge an overall rating of 2 out of 5, below the Iowa nursing home average of 3.1.
During the quarter, Southeast Iowa Regional Medical – Klein Center led as the county’s largest nursing home with 160 beds, while Oakview Nursing and Rehabilitation placed third with 60 beds.
Azria Health Prairie Ridge recorded zero fines or penalties in the first quarter of 2026.

Estimates from the U.S. Census Bureau indicate the nation’s population of older adults continues to rise as the number of younger individuals declines. From 2020 to 2024, Americans age 65 and older increased by 13%, while those under 18 decreased by 1.7%. The older adult population has seen significant expansion in the past century, from 4.9 million in 1920 to more than 61 million by 2024.
Research published in 2021 estimates that almost half of those age 65 and older—about 28 million people—may need long-term services and support. The same study suggests more than one-third could eventually require nursing home care.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
